Insurance

Please check with your health insurance or employee benefits for information about covered services. Before you come to your appointment, your insurance coverage is your responsibility to understand and bring.

Check with your insurance carrier or applicable departments (HR, Legal) at your company and ask:

  • Do I have mental health insurance benefits?

  • Does my policy cover remote, tele-health mental health appointments?

  • What is my deductible and has it been met?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount per therapy session?

  • Is approval required from my primary care physician?

Here are the insurance carriers where I am in network:

  • Blue Cross Blue Shield

I also accept self-pay. Note, some insurance companies may provide out-of-network (OON) benefits. If you wish to submit your sessions for potential OON coverage, I can provide you with a "superbill" you may submit for that purpose.

Note: Effective 1/1/22, a federal ruling went into effect called the “No Surprises Act,” protecting clients against surprise medical billing. This act requires all healthcare providers to provide a “Good Faith Estimate” (GFE) to any patient who is uninsured or who is insured but does not plan to use their insurance to pay for health care services. If applicable, your GFE will explain the cost of services that are reasonably expected for mental health care. It will not include any unknown or unexpected costs that may arise during treatment.

Payment Information

Cash or check are accepted for payment. All payments, including copays or co-insurance are due in full at the time of the appointment. A $25 fee will be added to checks that are returned NSF.

Cancellation Policy

If you miss your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay $100 for the missed session. This fee will not be covered by your insurance carrier. All cancellations are to made by calling my work number 515-259-9808. If you have a Monday appointment, cancellations must be received by the Friday before your appointment.

Confidentiality

Confidentiality is central to the relationship between a client and psychotherapist.  Confidentiality ensures that therapy is a safe and trusted space in which to share important, sensitive and private information.  At the commencement of therapy, I will provide you with a written copy of my confidential disclosure agreement.   

Exceptions to confidentiality are mandated by state law, and professional ethics. 

They are as follows:

  • Suspected past or present abuse or neglect of children, adults, and elders to the authorities, including the Department of Human Services and law enforcement, based on information provided by the client or collateral sources.

  • If I have reason to suspect the client is seriously in danger of harming him/herself or has threatened to harm another person.

  • If the client wishes to have your information shared with a third party such as a physician or attorney, which I will share only after obtaining written permission.

Because confidentiality is of critical importance, you may leave private information on my private office phone mail, but please do not include private information on my contact form here, or in email.
